Recently, on a popular entertainment program, a male celebrity in his 40s who is known for his youthful appearance was asked about the secret to looking young. He replied that it was because his lower eyelid fat repositioning had been performed successfully.

As a result, lower eyelid fat repositioning has once again come into the spotlight among middle-aged people who want a more youthful appearance. Why is this procedure considered a go-to anti-aging solution for the middle-aged?

To understand this, it is first necessary to look at the principle and purpose of lower eyelid fat repositioning. When the fat beneath the eyes protrudes or the hollow beneath the lower eyelid becomes deeper, dark circles can appear more pronounced. This can make the overall facial expression look dull and can also make the face appear tired and older.

Lower eyelid fat repositioning is a method that moves protruding fat beneath the eyes toward relatively hollow areas below it, helping to create a smoother under-eye contour and improve the appearance of dark circles. In this way, it can make the face look younger, brighter, and more refreshed.

For this reason, lower eyelid fat repositioning is regarded as one of the representative procedures in anti-aging eye surgery for middle-aged patients. In general, dark circles caused by under-eye fat tend to become more noticeable as aging progresses.

This can happen because the orbital septum that supports the fat beneath the eyes weakens, skin elasticity decreases as collagen and elastin decline, and changes in facial fat distribution can increase the accumulation of fat under the eyes. Lower eyelid fat repositioning may help improve dark circles that develop or worsen through these aging-related changes, which is one reason it has gained popularity as a procedure for a more youthful appearance in middle age.

Typically, lower eyelid fat repositioning takes about one hour and may be performed under local anesthesia or sedation. Suture removal is not required, and recovery usually takes about five to seven days.

After surgery, consistent cold compresses can support recovery, and patients should avoid bending the head forward for long periods or sleeping on the side. Strenuous exercise, sauna use, alcohol, and smoking should also be avoided. Light walking may help support recovery. Following post-operative recovery instructions can help improve the completeness and stability of the surgical result.

Dr. Kim Ju-chan of IBON Plastic Surgery in Busan explained, “Lower eyelid fat repositioning is a procedure that attracts interest not only from middle-aged patients who want a more youthful appearance, but also from younger people who want a bright and healthy-looking face. Dark circles can occur or worsen not only because of aging, but also because of genetic factors, lack of sleep, stress, excessive salt intake, and habits such as rubbing the eyes, so younger people are not necessarily exempt.”

He added that anyone considering the procedure should receive an accurate diagnosis and sufficient consultation, with a systematic plan based on the underlying cause of the concern.

Dr. Kim also noted that skin thickness, the amount of under-eye fat, and the severity of dark circles differ from person to person. Therefore, rather than choosing a clinic based only on cost or relying on unverified reviews, it is advisable to check whether the clinic can provide truly individualized, one-to-one surgery focused on the patient’s own condition. It is also important to confirm whether the medical team has the experience and expertise to offer a customized treatment plan.
